Coaching for Purpose™ - Policies and Paperwork
It is my goal at Coaching for Purpose to make the client/coach relationship as simple and clean as possible. For that reason, I have created links to the initial paperwork you'll need. Each document is a PDF so you will be able to open it with any computer. These documents require Adobe Reader. If you don't have Adobe Reader, click on the link below.

(You may also download the files into Microsoft Word if you'd like to fill in the forms online.)
Policies:
It is important to me as a coach that I model the balanced and fulfilled life I work on with my clients. For that reason, I outline here the policies that govern our coaching relationship.
- I work with clients on Wednesdays, Thursdays, and Fridays.
- I am available through email at tom@coachpurpose.com for questions or insights you might have in between sessions.
- Coaching is a longterm project that requires commitment and quality effort. My experience has been that clients do their best work if they commit to a six month stretch of coaching. However, I strongly suggest you plan on a minimum 3-month commitment. This includes 2 OR 3 60-minute phone meetings per month. After the initial 3-month commitment, we will work together on a month-to-month basis.
- Payment is due on the 25th day preceding the month of coaching. I accept checks and credit cards.
- I will do "make-up" appointments if given 48 hours advanced notice. Otherwise I will charge you 50% of the fee for that missed appointment.
- Appointments will start at the agreed-upon scheduled time. If you are late, the appointment will still end at the end of the scheduled hour so please be on time.
- Pricing:
- 3 or more sessions per month: $85/session
- 2 sessions per month: $95/session
- 1 session per month: $105/session
- Prepay for 3 months (9 sessions): $700
